What to bring
Having these items with you helps us verify benefits, review your records, and plan safely.
Photo ID
A driver’s license or other government-issued identification.
Insurance card
Your current dental and, if applicable, medical insurance cards so we can verify benefits.
Referral and records
Any referral slip, X-rays, or imaging from your dentist or orthodontist. If your referring office is sending records directly, let us know when you schedule.
Medication list
A current list of all prescription medications, over-the-counter medicines, and supplements you take, with dosages.
If a parent or guardian is accompanying a minor, please plan for them to stay for the consultation and consent discussion.

Health-history review
Your surgeon reviews your medical conditions, medications, allergies, and prior surgeries or anesthesia experiences. This shapes both the surgical plan and the comfort options that are appropriate for you, so please answer completely — including details that may not seem dental.
Imaging
We review any imaging your referring office sends. If additional or updated imaging is needed to plan your care, it may be taken at your visit. Detailed imaging helps your surgeon plan around your individual anatomy.

Common first-visit questions
Will I need someone to drive me?
Not for a consultation. If your treatment plan later includes sedation or anesthesia, your written pre-operative instructions will confirm whether you need an escort and what the arrangements should be.
Can treatment happen the same day as the consultation?
That depends on your situation, health history, imaging, and the preparation your procedure requires — your surgeon will tell you what is possible for your plan. Some visits are consultation only so that anesthesia preparation and benefits verification can be completed first.
What will it cost?
