Financial policy, privacy notice acknowledgment, and any procedure-specific consent your surgeon reviews with you in person.
Online forms
Secure online completion is planned. Until the secure channel is live, our team will help you complete paperwork at the office or by another arrangement when you call.
Protect your health information
Please do not send health details, photos, medication lists, or completed forms through email or general contact messages until our secure channel is confirmed. Call the office instead — our team will take your information safely or arrange paperwork at your visit.
