You do not need all of this — but each item helps us help you faster.
- What happened, and when the symptoms started
- Where the pain or swelling is, and whether it is getting worse
- Any recent dental or surgical work, here or elsewhere
- Current medications and allergies
- Your dentist's name, if you were referred or have one
- If a tooth was knocked out: keep it moist (in milk or saliva) and call immediately
