Emergency or routine?

Urgent patients get a phone-first path. Routine referrals follow the records checklist below.

Emergency referral

For acute infection, trauma, uncontrolled bleeding, or post-operative complications, call us directly so we can triage and see your patient promptly. Do not route urgent cases through written referrals.

Routine referral

For planned extractions, implant evaluation, pathology review, impacted canines, and other non-urgent care: send the records listed below by phone and we will contact your patient to schedule.

Records

What to include with a referral

Complete records let us schedule the right visit length and avoid repeat imaging.

  • Referral note

    The reason for referral, the teeth or sites involved, and any specific question you want answered.

  • Imaging, with dates

    Recent radiographs or CBCT volumes, labeled with acquisition dates, so we can judge whether new imaging is needed.

  • Relevant health history

    Medical conditions, current medications, anticoagulation status, and anything bearing on anesthesia planning.

  • Orthodontic plan, if applicable

    For impacted canines and surgical exposure cases: the treatment plan and desired eruption path from the orthodontist.

Communication

What you can expect from us

For you as the referrer

  • Acknowledgement that your referral was received and your patient was contacted. Turnaround expectation: we typically respond within one business day
  • A post-operative report after treatment, including findings, what was done, and any follow-up we recommend on your side.
  • A direct conversation with the treating surgeon whenever you want to discuss a case before or after referral.

For your patients

  • A calm, clearly explained consultation covering options, comfort and anesthesia choices, and costs before treatment.
  • Written pre- and post-operative instructions, with a direct line for questions during recovery.
  • A plan that respects your restorative or orthodontic treatment goals — we coordinate rather than redirect care.

How patient information is handled

Referral records are used only for evaluating and treating the referred patient and for coordinating care with your office. Until a secure, BAA-covered electronic channel is validated, we ask that records travel by phone or fax rather than email or web forms.
